Send us an email addressed to becky@akwaabacare.com and include the following information: your full name, date of birth, sex at birth, and a copy of your insurance card- front and back. We will create your patient account and reply to you with an invitation to the patient portal, where you will access the required intake forms and complete the registration process, and schedule your first appointment.
